Recovery from a brain injury is a long-term process. While most patients are discharged home after only a few weeks of inpatient rehab, ongoing services will be needed to continue the recovery process. The highly-skilled professionals at Frazier Rehab recognize how difficult it is for survivors and families to cope with the effects of brain injury. Your rehabilitation team will make recommendations to support your recovery.
Our outpatient brain injury rehabilitation services fall under two types of programs:
- Individual outpatient services designed to address specific therapeutic interventions
- A comprehensive community re-entry day program at the NeuroRehab Program
Outpatient rehabilitation services
Every brain injury doesn’t require the full spectrum of care. Frazier Rehab has an extensive outpatient rehab network that can provide expert therapy services based upon your individual needs.
Frazier Rehab proudly offers specialized therapies and evaluations throughout the region, including:
- Physical therapy
- Occupational therapy
- Speech-language therapy
- Assistive technology consultations
- Driver assessment and training
Individual treatment plans are developed in collaboration with the client and their support system. It is important to us that you feel involved and participate in your care. We will involve you in ongoing education and treatment planning throughout your course of therapy.
Frazier’s NeuroRehab Program is a comprehensive outpatient neuro-rehabilitation program for adolescents through adults who have sustained an acquired brain injury or another neurological injury, including:
- Traumatic brain injury
- Concussion (mTBI)
- Brain tumor
- Anoxic brain injury
The NeuroRehab Program is comprised of an interdisciplinary team of experts, including speech-language pathologists, occupational therapists, physical therapists and psychologists. Our team approach ensures we address all aspects of rehabilitation - physical, cognitive, vocational, social/interpersonal and emotional.
Individualized treatment plans are developed in collaboration with our patients, their support system and your payer source. Through innovative and adaptive therapies in real-world situations, our ultimate goal is to help patients become as independent as possible.
The NeuroRehab Program offers two types of programs:

- Individual outpatient services designed to address specific therapeutic intervention
A referral to the NeuroRehab Program ensures you get the unique, variable and individualized services you need to maximize your recovery. Services offered at the NeuroRehab Program include:
- Individual therapy
- Therapeutic groups
- Case management support services
- Cognitive skill-building and retraining
- Speech and language retraining
- Physical therapy
- Psychological support services
- Preparation for independent living
- Home evaluations and home management
- Prevention/wellness education
- Driving skills training
- School re-entry and IEP/504 collaboration
- Support groups for patients and caregivers
In addition to the services listed above, our interdisciplinary team of experts will provide you with additional recommendations and referrals that may promote your recovery. These recommendations may include, but are not limited to neuropsychological evaluation, vision specialist, specialized residential brain injury treatment program, rehab physician/physiatrist, home health therapies, skilled nursing facility placement or an adult day treatment program.
