Pre-employment Credential Verification and Kentucky Licensure Information
All new providers seeking employment by UofL Health and UofL Physicians must verify their credentials.
The Federal Credentials Verification Service (FCVS) is a fast and convenient way to submit and store core credentials that can be used to verify education and training information. By visiting fsmb.org/fcvs, you can complete your online application.
We encourage you to apply early as we cannot guarantee how long it will take for the FCVS to process your application. The credentials verification process is dependent on the timely and complete responses of medical schools and training programs.
Because unforeseen delays can occur during the credentialing process, FCVS encourages its applicants to complete their application at least two months before they need a FCVS profile submitted to the respective state medical board for licensure. Please keep all receipts and documentation of payment for reimbursement purposes upon your start date.
