The UofL Health Radiologic Technology Program application window opens mid-May through July 1 ONLY. Applicants will fill out the form below and upload their official transcripts during this process. Classes begin in October.

  • I understand that I must upload my official transcripts.
  • I understand that I must have all vaccinations listed.
  • I understand that I must take an entrance exam as part of the application process.
  • I understand that I will be interviewed by the radiology staff for enrollment if selected.
  • I understand that falsifying any statements on this application will cancel my application.
  • I understand that applying to the radiology program does not guarantee admission to the program.
