Our comprehensive care team specializes in heart, kidney, liver, lung and pancreas transplants. We also offer a dedicated Living Kidney Donor Transplant Program and provide comprehensive care through our UofL Health – Advanced Heart Failure Therapies clinic, in partnership with UofL Health – Jewish Hospital.

How Do I Make a Referral?

Referring yourself for an evaluation: Our transplant center allows individuals to submit a self-referral for a transplant evaluation for any of our organ programs, as well as the Advanced Heart Failure Therapies clinic. To make a self-referral, please select “Patient” from the first question’s drop-down menu.

Referring a patient: Our transplant center works closely with referring providers to ensure a seamless and supportive experience for all our organ programs and the Advanced Heart Failure Therapies clinic. For Referring Providers | Seamless Kidney and Pancreas Referrals with TxAccess

UofL Health – Trager Transplant Center has partnered with TxAccess, a digital platform designed to enhance continuity of care for transplant patients. TxAccess improves coordination between transplant centers, nephrology practices and dialysis facilities, ensuring seamless communication and real-time access to critical patient information.

This secure and user-friendly platform also allows you to submit new kidney and pancreas referrals electronically, exchange messages and documents linked to individual patients and track progress through every step of the transplant process.

After onboarding, you’ll have a personalized dashboard displaying the status of all patients you’ve referred to Trager Transplant Center, including upcoming appointments (and appointment history), outstanding tests and other information for insight into the patient journey with the transplant program — all directly synced from our electronic medical record.
