Referring Physicians
Thank you for referring your patient to UofL Health – Hand Care. Below is the contact information for scheduling new and return appointments, as well as a link to a Referral Fax Form. Please complete the Referral Fax Form with the patient’s information and a direct number back to your office. We will call the patient, schedule the appointment and then fax you back the appointment confirmation. If you prefer to call, please contact the schedulers directly to make an appointment.
