Hospitals, Medical Centers & University of Louisville Physicians
The Medical Records Request Form can be sent using the following three options:

Payments for ROI requests can be made online here.
You can reach the Health Information Management office at 502-217-1900 for continuity of care needs.
Status checks for medical records requests should be emailed to COCRequest@UofLHealth.org.
Responses will be received within 48 hours.
Request for Radiology Images (Films) Only

If you did not receive your imaging results, please reach out to your provider or you can request the images through your MyChart or submit a request through this form.
Contact the Radiology department for continuity of care needs.
Status checks for Radiology images requests should be directed to ROIRadiologyImages@UofLHealth.org. Responses will be received within 48 hours.
UofL Health offers two options for Radiology Images (Films): Change Healthcare Stratus and PocketHealth.
Change Healthcare Stratus
Access your radiology images for free.
- Complete the online form for Radiology Images(films). The ROI team will send an email for you to register with Stratus. Once you sign up, your images will be available online.
- Instantly download or share your records.
- Receiving physicians can then easily view or import these images in full diagnostic quality with no software or account setup needed.
- Call 502-217-1900 with issues accessing the images.
Pockethealth
With just a few clicks, securely access your imaging records from anywhere, on any device.
UofL Health patients can now access your medical imaging and reports with PocketHealth!

To access your records for FREE:
- Visit or call UofL Health at 502-217-1900, option #1 to receive your MRN.
- Go to PocketHealth.com/UofLHealth to sign up using your MRN.
- Once your records are ready to view, you’ll receive an email from PocketHealth with a secure link to view them.
- Instantly download or share your records.
- Receiving physicians can then easily view or import these images in full diagnostic quality with no software or account setup needed.
