This screening is on September 25, 2023 from 8:30 a.m. to 3:30 p.m. Call 502-852-6318 to schedule an appointment. All insurance will be filed with UofL Hospital. Proper ID and Insurance Card are required. Financial assistance is offered for uninsured patients who qualify. All COVID guidelines will be followed.
