Community Health Needs Assessments (CHNA) are required of nonprofits hospitals every three years. The last CHNA for each of the UofL Health hospitals was completed in 2022. The UofL Health – Organizational Development, Engagement and Inclusion Team is working with area health departments to complete the next assessment.

The purpose of the CHNA is to gain a thorough understanding of the community that we serve to inform our practices and services and break down barriers to care. Once we assess the needs, gaps in health and wellbeing are prioritized and strategies are developed with each of our seven hospitals.

We need your help to collect survey responses. Please take a few minutes to complete the online survey from Metro Louisville. The survey asks about your perceptions of the community related to health, health care, resources, priorities in the community and general health and well-being.

Survey responses will be included in our upcoming CHNA and Metro Louisville’s assessment report. The survey is confidential and identifying information will not be connected to responses to the survey.
